- Please download and print the Exceptions Form. (Adobe Acrobat Reader is required to read and print this document. To download a free version if you do not have Adobe Acrobat Reader click here)
- Complete the form by filling in the required information.
- Mail or fax the form to ProPay Canada. Mailing and faxing instructions are included on the form.
You should receive a response from ProPay within two to three business days after we receive your form.
