1. Please download and print the Exceptions Form. (Adobe Acrobat Reader is required to read and print this document. To download a free version if you do not have Adobe Acrobat Reader click here)
2. Complete the form by filling in the required information.
3. Mail or fax the form to ProPay Canada. Mailing and faxing instructions are included on the form.

You should receive a response from ProPay within two to three business days after we receive your form.
